Marin Community Clinics, founded in 1972, is today a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty, and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. Marin Community Clinics is looking for a mission-driven Associate Case Manager to join our dynamic, multidisciplinary Complex Care team to advance our goal of improving health outcomes for individuals with complex medical, behavioral health, and psychosocial needs. Our Complex Care team is committed to increasing access and reducing barriers to care for Marin’s most vulnerable patients through person-centered, supportive case management services. Working collaboratively with primary care, behavioral health, and other members of the clinical care team, the Associate Case Manager works as a member of an expert team of professionals to provide patient-centered, outcome-driven care that promotes independence and dignity.

Responsibilities

  • Support a minimum caseload of 30–40 Complex Care patients.
  • Implement comprehensive patient care plans in partnership with patients.
  • Triage and respond to tasks/care needs.
  • Assist patients to complete necessary forms/applications.
  • Work with patients to address non-medical factors that affect overall well-being.
  • Assist patients in getting connected to community resources.
  • Assist patients to schedule/attend medical, behavioral health, and specialist appointments.
  • Administrative support including records retrieval and review, eligibility screening, goal tracking, and more.
  • Assist patients with coordinating all necessary care.
  • Conduct home/community visits as needed.
  • Consult/coordinate with interdisciplinary care teams, including behavioral health, care navigation MAs, nursing, medical providers, and external partners.
  • Document all patient interactions and services using MCC’s electronic health record and secondary database.
  • Attend internal and external meetings (e.g., case conferencing, administrative).
  • Attend trainings such as health coaching, engagement techniques, etc.
  • Provide timely and accurate reporting of data as requested by MCC programs.
  • May travel between clinics or meet patients in the community as needed.
  • Provide thoughtful feedback regarding case management services and quality improvement measures.
  • rovide services to satellite clinics as needed; may be asked to work a variable schedule during extended-hour clinics (e.g., 10am–7pm on one or more days).
  • Other duties may be assigned.

Qualifications

Education and Experience:

  • MA, CHW, Bachelor of SW, or equivalent case management experience required.
  • 1–2 years’ experience providing support to individuals with chronic and complex behavioral health needs, complex medical conditions, and/or individuals who use substances highly preferred.
  • Experience/familiarity with housing resources and housing navigation in Marin highly preferred.
  • Knowledge of state regulations and legal/ethical standards related to patient rights and client/patient confidentiality required.
  • Training in motivational interviewing highly desired or willingness to learn.

Required Skills and Abilities:

  • Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports and business correspondence in English.
  • Must be able to communicate with patients and partners in Spanish: effectively present information and respond to questions from clients, customers, and the public in Spanish.
  • Must be able to conduct clinical assessment interviews and case management interventions with patients in Spanish.
  • Willing to drive to other MCC clinic locations in Marin County.

Computer Skills:

  • To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs and how to conduct searches on the Internet.

Certificates, Licenses, Registrations:

  • BLS Training Current.

Physical Requirements and Working Conditions:

  • Prolonged standing and walking.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Will drive among clinic locations as needed.
  • Fulfill immunization and fit-for-duty regulatory requirements.
