Overview

Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Clinics regularly receive national awards from the Health Resources and Services Administrations (HRSA). Our Mission is to promote health and wellness through excellent, compassionate care for all.

 

Under the supervision of the Associate Dental Director, the Care Navigator, Dental is responsible for assisting dental patients with a variety of questions and concerns in determining treatment needs. These duties include helping patients compile the right documents and communicating with Dental professional to determine the right place for patients to receive care.

Responsibilities

  • Establishes a dental home for patients without one.
  • Collaborates with the medical department staff to facilitate a warm handoff for dental care.
  • Assists during Tele-dentistry appointments as needed.
  • Assists dentist with dental exam/fluoride application for infants/toddlers without dental home during patient well check visit.
  • Re-establishes patients in our dental system.
  • Collaborates with the dentists during school screening sessions, assisting the dentists, making appointments and referring patients to the clinics for urgent care.
  • Produces daily reports on potential fluoride applications etc.
  • Develops expertise in using scheduling and telephone system.
  • Screens calls from other medical facilities and agencies.
  • Has basic knowledge of all available dental insurance and funding programs, to assist patients with accessing care and appropriate appointments.
  • Proactive management of appointment schedule-- call patients with appointment later in the day as slots becomes available due to cancellations and no-shows.
  • Participates in Quality Improvement activities (log for tracking capacity).
  • Reminds patients to bring all medications to visit.
  • Should focus on assessing and resolving patients’ needs efficiently in a care team setting.
  • Uses scripts and tools to ensure most appropriate scheduling of appointments with correct provider within correct time.
  • Never give dental advice to patients and direct all dental advice questions to Dentist.
  • Provides patients with health center directions; confirms time, location, and appointment.
  • Provides patient with parking advice or instructions.
  • Reminds patient to bring: Proof of Residency for new patients, proof of income and/or insurance. If insurance bring insurance card and photo ID.
  • Assesses need for interpretation at visit (document in details).
  • As time permits, enters patient registration information into Electronic Health Record Software.
  • Assesses literacy level, asks patient: “will you need help filling out paperwork ‘
  • Ensures customer satisfaction by proactively delivering problem solving ability.
  • Enhanced confirmation calls- including reminders of bringing meds, etc.
  • Other responsibilities as assigned by the Associate Dental Director
  • Works on special projects as assigned by the Chief Dental Officer.

Qualifications

Education and Experience:

  • High School Diploma or Equivalent (GED) required.
  • Project coordination experience preferred.
  • Experience with EPIC Electronic Health Records preferred.

Required Skills and Abilities:

  • Adheres to strict HIPAA and confidentiality laws regarding health information protections.
  • Ability to speak, read, and writes in English and Spanish required.
  • Ability to work independently and exercise good judgment with appropriate supervision.
  • Ability to maintain strict confidentiality regarding patient information.
  • Ability to understand complex concepts and convey them in a manner that is easily understood by layperson.
  • Ability to manage and prioritize communications from multiple staff, organizations, patients, etc., at the same time.
  • Ability to organize and review substantial amounts of data in an organized and efficient manner.
  • Ability to proofread, correct and review forms and data for scanning and/or entry.
  • Basic knowledge of Microsoft Office software required: Outlook, Word, and Excel.
  • Ability to work independently and work positively as a collaborative team member.
  • Ability to represent clinic in professional manner and handle patient issues with sensitivity and confidentiality.
  • Ability to meet deadlines and manage changing priorities.
  • Excellent oral and written communications skills.
  • Ability to wort at any of the MCC clinics, as needed.

Physical Requirements and Working Conditions: 

  • Prolonged periods of sitting at a desk and working on a computer.
  • Use of mouse, keyboard, and headset.
  • Moderate to loud noise and intermittent interruptions.
  • Must be able to lift to 15 pounds at times.
