Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. Using a trauma-informed approach, the recovery specialist serves as a care manager for MCC patients identified with Opiate Use Disorder (OUD) or other Substance Use Disorders (SUD) and/or other comorbid mental health conditions. The recovery specialist will work closely with primary care and, specifically, Medication Assisted Treatment (MAT) providers as part of the larger “hub and spoke” model with Marin Treatment Center. The Recovery Specialist, rooted in a commitment to recovery from addiction, will focus on assessment, education, brief intervention, and referral to treatment (including both SUD treatments and mental health treatment) for those patients receiving MAT. The recovery specialist will work closely with MCC’s addiction specialist MD and track a panel of MAT (and other substance using) patients from a population-based care approach and help ensure no one "falls through the cracks”. The Recovery Specialist may provide brief psychotherapy and/or crisis intervention to patients with comorbid mental health concerns. Navigation of the system of care will include brokering and maintaining relationships with external county substance abuse and mental health resources (for higher-level care) as well as patient education and linkage to integrated services within MCC.

Responsibilities

  • Provide psychosocial assessment and case management services to MCC patients receiving MAT treatment and/or those with identified OUD/SUD and/or concurrent mental health diagnoses.
  • Maintain and track services for panel of patients identified with OUD/SUD.
  • Ability to identify psychosocial needs and assign a diagnosis based on DSM criteria.
  • Provide crisis intervention as indicated.
  • Provide consultation to medical providers regarding the behavioral health components of a patient’s presentation. Develop collaborative plans to address psychosocial needs.
  • Work with patients in their preferred language (primarily English and Spanish), using a language line as needed. 
  • Use the “warm hand off” method (immediate brief assessment and intervention) for assisting patients who have identified behavioral health/SUD issues at the time of their medical visit.
  • Develop and co-facilitate Chronic Pain Group in collaboration with MCC’s Addiction Medicine Specialist.
  • Demonstrate a willingness to acquire specific knowledge of opiate use disorder and MAT.
  • Demonstrate a willingness to acquire general knowledge of psychopharmacology and engage in ongoing learning about new medications and treatments commonly used for mental disorders, substance abuse, and general health problems.
  • Use the Clinic’s electronic health record (EHR) to document clinical work, produce billing, collect service data, and maintain a schedule, as well as to communicate information to other clinic providers.
  • Assist in developing and maintaining procedures and protocols to ensure the Behavioral Health program’s relevance and effectiveness for patients and clinic staff.
  • Work closely with behavioral health, care navigation and medical providers.
  • Attend behavioral health meetings, care navigation meetings, medical provider meetings, and all staff meetings, as scheduled.
  • Provide frank and considered opinions regarding addiction services and quality improvement measures.
  • Provide Behavioral Health care to satellite clinics as needed; may be asked to work variable schedule during extended-hour clinics and/or via tele-health (e.g., 10am-7pm on one or more days).
  • Other duties may be assigned.

Qualifications

Education and Experience:

  • Licensed Clinical Social Worker (LCSW) or PsyD/PhD.
  • Demonstrated ability to work effectively on a team, show initiative, flexibility and enhance a professional culture of trust and good will.
  • Previous clinical experience should include assessment and diagnosis with substance-using population, intensive case management, group work, and crisis intervention.
  • Knowledge of state regulations and standards related to client treatment, patient rights, and client/patient confidentiality required.
  • Expert in motivational interviewing and other evidence-based modalities for working with substance using individuals.
  • Demonstrated familiarity with SUD, chronic pain, and addiction. Familiarity with psychopharmacology is helpful.
  • Demonstrated ability to work effectively on a team, preferably in a primary care environment.
  • Ability to see 8-10 patients per 8-hour shift.

 

Required Skills and Abilities:

  • Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports and business correspondence in English.
  • Spanish preferred but not required. Must be able to use telephonic translation service.

Computer Skills:                                                    

  • To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.

Certificates, Licenses, Registrations:

  • CPR Training Current.
  • Valid Driver’s License.

Physical Requirements:

  • Prolonged standing and walking.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Ability to drive to clinics and other locations in the community.
