The Complex Care Team Lead works to advance our goal of improving health outcomes for individuals experience complex medical, behavioral health and psychosocial needs. The Complex Care Team Lead works collaboratively with primary care, behavioral health, and other members of the clinical care team. This position provides leadership and support to a team of Associate Case Managers to ensure the highest level of patient care and staff satisfaction. The Complex Care Team Lead is a patient-facing role that supports engagement and builds trust with patients with complex medical and psychosocial needs.
