Overview

Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Clinics regularly receive national awards from the Health Resources and Services Administrations (HRSA). The Registered Nurse (RN) is responsible for performing floor procedures, triaging walk-in and late patients, and performing nurse case management functions. They perform office duties, such as, assessing labs and consulting as needed, answering the RN phone line, and setting up appropriate appointments with patients and outside providers. The RN also provides health education, phone triage and advice, and other functions to ensure a smoothly functioning clinic.

 

Responsibilities

  • Follows up on phone calls within one hour and charts follow up as requested by providers by the end of the shift.
  • Triage of patients per protocol including walk-ins and late patients and by telephone.
  • Performs floor procedures, including but not limited to; catheterizations, phlebotomy and IV therapy, wound care, assist providers and perform procedures accurately and thoroughly per protocol and as demonstrated on the skills assessment.
  • Administers injectables including insulin, antibiotics, toradol, pain medication, kenalog, and immunizations following standard procedures and as demonstrated on the skills assessment. Administer and monitor oxygen and nebulizers per protocol and as demonstrated successfully during the skills assessment.
  • Ascertains pre-op needs and visit per type of surgery and schedule appointments accordingly. Completes pre-op checklist prior to appointment; arranges appropriate labs and tests.
  • Provides patient education as necessary, including but not limited to: Asthma, diabetes, glucose monitoring, medications, wound care, signs and symptoms to report of disease process. Provides appropriate handouts and documentation.
  • Responds to and manages MD STATS as per protocol. Maintains crash cart and orders supplies.
  • Reviews and prioritizes incoming labs throughout the day and manages follow up as indicated before the end of the RN shift.
  • Responds to patient requests for lab results, medical questions and questions and concerns regarding disease processes. Seeks additional information and asks for assistance as needed.
  • Actively assist MAs in maintaining efficient flow rooming patients, completing MA duties, and directing flow as necessary.
  • Manage and monitor patients on anticoagulants. Track results successfully and observe trends for PT/INRS. Manage and monitor other specialty meds such as syna gcs.
  • Performs all MA duties as necessary to provide breaks or fill-in when short staffed. Refer to the MA job description -- standards. Maybe just quantify by stating able to work independently with provider.
  • Provides basic health care information and advice to clients based on RN scope of practice.
  • Performs suture removal.  
  • Checks blood pressure.
  • Performs nurse case management. Monitor Coumadin therapy and PT/INRs, manage other meds as directed by provider. Coordinate pre-ops. Work with Marin Home Care and other services and providers to coordinate patient care.
  • Carries out MD orders accurately and in a timely manner for RN follow up
  • Calls patient and arranges for repeat labs, when necessary
  • Maintains crash cart and oxygen tanks per protocol. Log crash cart inventory and supplies ordered monthly.
  • Perform pediatric assessments, clinical skills and treatments per protocol. Assess, triage or room a patient, including but not limited to; catheterization, immunizations, nebulizer treatments etc.
  • Performs adult assessments, clinical skills and treatments per protocol. Assess, triage or room a patient, including but not limited; to nebulizer treatments, EKGs, Injections, wound care, etc.
  • Maintains valid and active CPR for the health care professional. Renews card prior to expiration.
  • Maintains valid California RN license. Renews and provides renewal prior to expiration.
  • Oversees inventory and ordering of immunizations, medications, and medical supplies. Ensures supplies are available.
  • Proficiently uses medical management system. Able to schedule appointments, research patient information, and check provider appointments.
  • Assists the Nursing Admin assistant with medication refills. Monitor requests for narcotics and audit charts and patient requests for narcotics to assure adherence to policies.
  • Be resource for Medical Assistants in decision making and patient safety.
  • Care Team Responsibilities as outlined in the Care Team Patient Center Medical Home procedures.
  • Other duties as assigned.

Qualifications

Education and Experience:

  • Associate or BA/BS degree required.
  • 2 plus years nursing experience in a comparable work environment, or equivalent.
  • Valid California license as a registered nurse.
  • Current CPR/BLS Certification.
  • Experience with EPIC Electronic Health Records preferred.

Required Skills and Abilities:

  • Excellent verbal and written skills.
  • Must be able to work independently, with written protocols.
  • Must be sensitive to ethnic and cultural differences and their relevance to health care.
  • Bilingual English-Spanish preferred.

Physical Requirements and Working Conditions:

  • Prolonged periods of typing and sitting at a computer.
  • Must be able to lift up to 15 pounds at a time.
