Overview

The Care Navigator MA will perform a wide range of duties in the medical office. Tasks primarily (although not limited to) help patients navigate the support services in the community and apply for programs needed. Assist as a Medical Assistant in examination and treatment of patients under the direction of a Physician, Physician Assistant and/or Family Nurse Practitioner. Care Navigator MA will have direct contact with patients and work closely with the care team. Care Navigator MA is under the direction of the Medical Assistant Supervisor and Clinic Director.

Responsibilities

Under general supervision, the Care Navigator MA primary responsibilities include but are not limited to:

  • Escorts patients to the exam room, interviews patients, measures vital signs and records information on patients’ charts.
  • Prepares treatment rooms for examination of patients.
  • Follow policy and procedures including universal precautions as defined by OSHA’s guidelines on Bloodborne Pathogens and Hazardous Materials standards.
  • Practice good people skills to maintain good public relations with patients and with fellow co-workers.
  • Maintain patient confidentiality in accordance to department policy and HIPPA guidelines.
  • Attend and participate in required meetings, training and educational programs.
  • Performs administrative responsibilities, including checking eligibility, verifying demographics, assisting patients with filling out necessary paperwork, and making up follow-up appointments.
  • Stays current in resources in the community.
  • Perform other related work as assigned.

 

  New patient management:

  • Call new patients (PHC) to welcome/schedule first visit.
  • Assign PCP and needed health maintenance screenings
  • Follow up/ track “no shows” for first appointment

Patient assistance medication program management

 

  • Track and assist patient will needed forms.

Complete/track TARs

  • Request Prior Authorizations and TARs sent by the pharmacy so patients can get ordered medications in a timely manner.

Qualifications

                                     

  • One year certificate from college or accredited technical Medical Assisting program; or one to three months related experience and/or training; or equivalent combination of education and experience.
  • At least 1 or 2 years work experience in medical clinic setting, Federally Qualified Health Center a plus.
  • Medical Assistant Certificate from an accredited school required
  • Current CPR certification required
  • NextGen experience a plus but not required.
  • Electronic Health Records experience a plus.
  • Bilingual (Spanish/English) language skills required
  • Must be able to work flexible hours.
  • Ability to work in fast pace medical environment.
  • Must have strong computer experience with MS Office
  • Ability to provide excellent customer service to patients and outside customers in person and via phone.
  • Must be able to multi-task duties and carryout projects timely.
